The registrant must have a physical presence in one of New York City’s five boroughs: the Bronx, Brooklyn, Manhattan, Queens or Staten Island.

Please specify which of the following categories applies to the registrant:
  1. Organisation or entity with a physical address in New York City, or
  2. Individual whose primary residence is in New York City.
Please note that P.O. boxes are not accepted as proof of physical presence.
